DEPOSIT POLICY
I understand that all deposits are non-refundable and non-negotiable.
This response is required to reserve.
LATE FEE POLICY
A $15 late fee applies at the 10-minute mark. Between 10 and 15 minutes late (past the grace period), a $25 late fee applies. More than 15 minutes late will result in your appointment being automatically cancelled and your deposit forfeited. You must be seated in the chair before this time.
PHOTO RELEASE
I agree to let NHHANCD Society share client photos of me on the @nhhancdsociety Instagram. If I do not agree, I will inform Najiyyah Jones prior to any photos being taken.
ALLERGIES
Please list any allergies that may have an effect on makeup application. If none, please insert “N/A”.
SKIN HEALTH
I understand that Najiyyah Jones will deny clients and cancel my appointment if I am experiencing severe acne, blisters, bloody scabs or any open wounds on the skin.
CANCELLATION POLICY
Cancellations made within 24 hours of your appointment will result in a forfeited deposit plus 50% of the remaining balance due.
REFERRAL
Were you referred by a previous client of mine or a friend? If so, please share the client’s name below.
BIRTHDAY
Is it your birthday? If so, please enjoy $10 off your service using the code “Birthday” (must show proof).
